Turning In Forms

You will need to hand in payment, permission/medical form, and a copy of your passport to complete the registration. The permission/medical form can be downloaded from youthscf.com/retreat/permission. You can turn in the forms at any one of the following locations:

SCF locations:

  • Saturday meetings (SOAR 24/7 – Youth Base)
  • Sunday meetings (SOAR 3D – Hengshan)
  • Hongqiao Youth Service (AIR)
  • During the week – SCF office (close to Yan’An West Rd – Gubei Carrefour) 水城南路89号 美丽华花园 7栋
    ShuiCheng Nan Rd. Shang-Mira compound # 89, Unit 7 (During office hours; please call to check if there are people there by the time you plan to come by. Phone: 32522362 ext 106)

Your payment, permission/medical form, and passport copy must be turned in no later than April 1st or there will be a 50 RMB late fee.

Remember, there are five things to do when turning in payment:

  1. Turn in payment according to sign up date
  2. Include copy of passport
  3. Include permission form
  4. Put all three in an envelope
  5. Put your name on the envelope
